- As a courtesy to the next person, patients are urged to arrive on
time for their appointment.
- Please help us better serve you by canceling appointments at least
24 hours in advance.
Payment Policy Guidelines:
- We do not participate with Medicare, Medicaid or any Private Health Insurance
plan. WHY?
- Patients/guardians are financially responsible for all charges, regardless
of third-party involvement.
- Full payment is due at time of service.
- We accept cash, checks, Visa, Master Card, Discover and Amex.

Responsibilities:
- You have a responsibility to provide accurate and complete information about
your health history and contact information. If any information changes, you
should inform this practice immediately.
- Your health insurance coverage, if any, is a contract between you and your
health insurance plan. You are responsible for understanding your health insurance
coverage and for submitting claims to your insurance carrier. Any coverage
or payment dispute is a matter between the insurance policyholder and the insurance
company. If a payment is sent to us by your insurance carrier in error, we
will forward it to you.
